ఈసీజీని పరీక్షించి.. ఒక వ్యక్తి గుండె సామర్థ్యం ఎలా ఉందనేది ఏఐ సాయంతో 92-93 శాతం కచ్చితత్వంతో గుర్తించడం సాధ్యమేనని నారాయణ హృదయాలయ ఛైర్మన్, ప్రముఖ గుండె శస్త్రచికిత్స వైద్యనిపుణులు డాక్టర్‌ దేవిశెట్టి స్పష్టం చేశారు. ఈసీజీని ఏఐతో విశ్లేషించడం ద్వారా సడన్‌ కార్డియాక్‌ అరెస్టును కూడా ముందస్తుగా గుర్తించగలిగే అవకాశం ఉంటుందని అన్నారు. డిజిటల్‌ ప్లాట్‌ఫాం ద్వారా అతి వేగంగా సమాచార మార్పిడి జరుగుతుందని, ఏఐ సాంకేతికతను వినియోగించడం ద్వారా వేగం, కచ్చితత్వంతో కూడిన చికిత్సను అందింవచ్చని అభిప్రాయపడ్డారు. వచ్చే పదేళ్లలో ఇది కీలకపాత్ర పోషిస్తుందని, చికిత్సలో సెకండ్‌ ఒపీనియన్‌ తీసుకోవాల్సిన సందర్భాల్లో దాని నుంచే స్వీకరించే పరిస్థితి ఉంటుందని తెలిపారు. అయితే అది స్మార్ట్‌ అయినా.. దానికంటే స్మార్టర్‌ డాక్టరే అని.. అంతిమ నిర్ణయం వైద్యుడిదే అని డాక్టర్‌ దేవిశెట్టి స్పష్టంచేశారు. వైద్యరంగంలో కృత్రిమ మేధ పాత్రపై ‘ఈనాడు ప్రత్యేక ప్రతినిధి’కి ఇచ్చిన ఇంటర్వ్యూలో ఆయన పలు అంశాలను వెల్లడించారు.
మెడికల్‌ ఎర్రర్‌ను ఏఐతో తగ్గించడం సాధ్యమవుతుందా?
మనం ఏం ట్రీట్‌మెంట్‌ ఇచ్చినా.. ప్రతి ఒక్కటీ రికార్డవుతుంది. ఐసీయూలో కిడ్నీ సమస్య బార్డర్‌లైన్‌లో ఉన్నవారికి ‘జెంటామైసిన్‌’ ఇంజెక్షన్‌ ఇచ్చామనుకోండి.. ఇది ‘నెఫ్రోటాక్సిక్‌’ ఇంజెక్షన్‌. కిడ్నీని దెబ్బతీస్తుంది. డాక్టర్‌ పేపర్‌పైన రాస్తే.. నర్సు ఇస్తారు. అదే ఏఐతో ఉన్న స్మార్ట్‌ డిజిటల్‌ ప్లాట్‌ఫాంలో అయితే.. మీరు ‘జెంటామైసిన్‌’ అని రాయడం పూర్తి చేయగానే అది ‘పేషెంట్‌ క్రియాటిన్‌ 2.5 ఉంది.. ఇవ్వడానికి వీలు లేదు’ అని చెబుతుంది. అది ఏఐకి ఉన్న పవర్‌. పేషెంట్‌ హెల్త్‌కేర్‌ను ఇది సురక్షితం చేస్తుంది.
నారాయణ హృదయాలయ కొన్నేళ్లుగా ఏఐని వినియోగిస్తోంది కదా? మీ అనుభవాలేంటి
ఏఐకి డిజిటల్‌ సమాచారం అవసరం. స్ట్రక్చర్డ్‌ డేటా కూడా కావాలి. పేషెంట్‌కు సంబంధించి ఏం జరుగుతుందో తెలుసుకోవాలంటే.. హెల్త్‌కేర్‌లో అత్యంత కీలకమైనది డయాగ్నొసిస్‌. మానవ మేదస్సు ఒకసారి పది నుంచి 15 డేటా పాయింట్లను పరిశీలించగలదు. కానీ ఏఐ.. 60 నుంచి 100 డేటా పాయింట్లు ఉంటే కూడా విశ్లేషించగలదు. మా ఆసుపత్రిలో ఎనిమిది సంవత్సరాల క్రితమే డిజిటల్‌ ప్లాట్‌ఫాం ప్రారంభించాం. 50 మంది సాఫ్ట్‌వేర్‌ ఇంజినీర్లతో కూడిన నారాయణ హృదయాలయ బృందం.. ఏఐ టూల్స్‌ తయారు చేసింది. మా టీములోని 20 మంది డాక్టర్లకు ఈ యాప్‌ ఉంటుంది. ఉదాహరణకు ‘డెడ్లీ బ్యాక్టీరియా’ ఉన్నట్లు తేలగానే డ్యూటీలో ఉన్నా లేకున్నా వైద్యులకు సమాచారం వెళ్లిపోతుంది. పేషెంట్‌ బెడ్‌ దగ్గర ఉన్న డాక్టర్‌ కచ్చితంగా చూస్తారు. అవసరమైతే మిగిలిన వైద్యులూ స్పందిస్తారు. వెంటనే కార్యాచరణ ప్రారంభిస్తారు. అలాగే ఐసీయులో కొన్ని గంటలకోసారి బ్లడ్‌గ్యాస్‌ పరీక్షలు చేస్తారు. ఈ పరీక్ష ఫలితాలు మా టీములోని 20 మంది డాక్టర్లు, నర్సుల ఫోన్‌లో ఉన్న యాప్‌నకు వస్తాయి. మామూలుగా కొద్దిమంది నర్సులు మాత్రమే ఈ రిపోర్ట్‌ను ఇంటర్‌ప్రిటేషన్‌ చేయగలరు. కొద్దిగా సంక్లిష్టమైనది. మా ఆసుపత్రిలో బ్లడ్‌ గ్యాస్‌ రిపోర్టుతో పాటు.. ఏఐ ద్వారా ఇంటర్‌ప్రిటేషన్‌ కూడా వస్తుంది. దాంతో ప్రస్తుతం అంతా నిపుణులయ్యారు. వైద్యులను, నర్సులను ఏఐ పనికిరాకుండా చేస్తుందనే వాదన ఉంటే.. దానికి భిన్నంగా ఏఐ వల్ల స్మార్టర్‌గా తయారయ్యారు. స్మార్ట్‌ ఈఎంఆర్‌ వల్ల మెరుగైన స్మార్టర్‌ డయాగ్నొసిస్‌ వస్తుంది. సమీప భవిష్యత్తులో సుమారు ఐదు నుంచి పది సంవత్సరాల లోపు కచ్చితంగా ఏఐ నుంచి సెకండ్‌ ఒపీనియన్‌ తీసుకోవడం చట్టపరంగా కూడా తప్పనిసరనే పరిస్థితి వస్తుంది.
గుండె సమస్యలను కూడా ఏఐ ద్వారా ముందస్తుగా గుర్తించవచ్చా?
కార్డియాక్‌ అరెస్టుకు ఆరు నుంచి ఎనిమిది గంటల ముందు కొన్ని సిగ్నల్స్‌ కనిపిస్తాయి. హార్ట్‌ రేట్‌ పెరుగుతుంది. బీపీ తగ్గుతుంటుంది. మూత్రం ఔట్‌పుట్‌ క్రమంగా తగ్గుతూ వస్తుంది. కార్డియాక్‌ అరెస్టుకు ముందు 67 రకాల అంశాలకు సంబంధించి డేటా పాయింట్లను విశ్లేషించాల్సి ఉంటుంది. ఇక్కడ ఏఐ కీలకంగా పని చేస్తుంది. మానవ మేదస్సు పనిచేసే దానికంటే భిన్నంగా పనిచేస్తుంది. మా బృందం ఓ అప్లికేషన్‌ను తయారు చేసింది. ఈసీజీని చూసి ఏ డాక్టరైనా ‘గుండె పనితీరు సామర్ధ్యం(ఎజెక్షన్‌ ఫ్రాక్షన్‌)’ చెప్పగలరా? ఆ సైన్స్‌ ఇంకా లేదు. కానీ ఏఐకి మా సిస్టమ్‌లోని లక్షల ఈసీజీలతో యాక్సెస్‌ ఉంది. సేమ్‌ పేషెంట్స్‌ ఎజెక్షన్‌ ఫ్రాక్షన్‌తో కూడా యాక్సెస్‌ ఉంది. కాబట్టి ఎజెక్షన్‌ ఫ్రాక్షన్‌ గురించి దాదాపుగా చెప్పగలదు. మా అధ్యయనంలో 92 నుంచి 93 శాతం కచ్చితత్వం ఉన్నట్లు తేలింది. ఎజెక్షన్‌ ఫ్రాక్షన్‌ తక్కువగా ఉంటే.. పేషెంట్‌కు ఏదో రకమైన హార్ట్‌ అటాక్‌ లేదా ఏదో జబ్బు ఉన్నట్లు. ఆ పేషెంట్‌కు తదుపరి పరీక్షలు చేయాల్సి ఉంటుంది. ఏఐ టూల్స్‌తో కార్డియాక్‌ విశ్లేషణ జరిగితే.. కనీసం 70 నుంచి 80 శాతం కార్డియాక్‌ అరెస్టులు ‘యాంటిసిపేటెడ్‌ కార్డియాక్‌ అరెస్టులు’గా మారతాయి. ఇది ఐసీయూ మేనేజ్‌మెంట్‌లో గేమ్‌ఛేంజర్‌ అవుతుంది.
ఏఐ సూచించిందే అంతిమమా?
ఏఐ.. టూల్‌ మాత్రమే. అయితే వైద్యుడికి అన్నిసార్లు సరిపోను సమయం ఉండదు. ఉదాహరణకు యాంజియోగ్రామ్‌ చూసినపుడు హార్ట్‌సర్జన్‌గా నా కోణం అంతా.. బైపాస్‌ సర్జరీ వైపే ఉంటుంది. అదే యాంజియోగ్రామ్‌ను కార్డియాలజిస్ట్‌ చూస్తే.. యాంజియోప్లాస్టీ గురించి ఆలోచిస్తారు. వీలుకాదనుకుంటే బైపాస్‌కు పంపుతారు. అంతిమంగా ఏం చేయాలో నిర్ణయించే ఏజెంట్‌ ఏఐ కాదు. అదెప్పటికీ జరగదు. అన్ని వేళలా డాక్టర్‌ మాత్రమే నిర్ణయిస్తారు.
వైద్య రంగంలో ఏఐ వినియోగం ఎలా ఉండాలి?
చిన్న పిల్లలు హోంవర్క్‌ చేయాలి. చాట్‌ జీపీటీని అడిగి రాస్తే.. అది దుర్వినియోగం చేయడమే. అలాగే కొందరు వైద్యులు వాళ్ల బ్రెయిన్‌ను వినియోగించకుండా ఏఐ ఏం చెబితే అది చేస్తున్నారు. అలాకాకుండా ఏఐ విశ్లేషణను అర్థం చేసుకొని వాస్తవ పరిస్థితులకు అన్వయించుకొని వైద్యులు నిర్ణయాలు తీసుకోవాలి.
ప్రధాన నగరాల్లో ఉండేవారికి తప్ప.. గ్రామీణ ప్రాంతాల్లోని పేషెంట్లకు ఏఐ పెద్దగా ఉపయోగపడదనే అభిప్రాయాలపై మీరు ఏమంటారు?
భారీగా డిజిటల్‌ హెల్త్కేర్, డిజిటల్‌ ప్లాట్‌ఫాంపైన వ్యయం చేయగలిగిన పెద్ద ఆసుపత్రులకు మాత్రమే ప్రస్తుతానికి ఏఐ పరిమితమైంది. పటిష్ఠమైన డిజిటల్‌ ప్లాట్‌ఫాం లేని ఆసుపత్రులు ఏఐని వినియోగించలేవు. సమయం పడుతుంది.
ఏఐ తప్పుడు వైద్యాన్ని సూచిస్తే ఎవరు బాధ్యులు?
వైద్యుడిదే బాధ్యత. గుర్తుంచుకోవాల్సింది ఏంటంటే.. ఏఐ.. వైద్యులకు సహాయపడే టూల్‌ మాత్రమే. వైద్యులు తప్పు చేయకుండా నివారిస్తుంది. మన మైండ్‌కు రాని ప్రత్యామ్నాయ ట్రీట్‌మెంట్‌ను సూచిస్తుంది. ప్రజలు అర్థం చేసుకోవాల్సింది ఏంటంటే.. ఏఐ వల్ల పేషెంట్లకు సురక్షితమైన హెల్త్కేర్ అందుతుంది.
వైద్య చికిత్సలో ఎలక్ట్రానిక్‌ మెడికల్‌ రికార్డులు (ఈఎంఆర్‌) విధానానికి ఉన్న ప్రాధాన్యం ఏమిటి?
అమెరికన్‌ ఆసుపత్రుల్లో చేరడమంటేనే.. సురక్షితమైన ఆసుపత్రుల్లో చేరినట్లుగా పలువురు భావిస్తారు. అలాంటి చోట కూడా మెడికల్‌ ఎర్రర్‌కు ఆస్కారం ఉంటోంది. ఉదాహరణకు అక్కడ ఓ ఆసుపత్రిలో 200 మంది పేషెంట్లు ఒక రాత్రి ఉంటే.. అందులో ఒకరు మెడికల్‌ ఎర్రర్‌తో చనిపోతున్నారు. అమెరికాలో కూడా చాలా హాస్పిటల్స్‌లో అత్యాధునిక సాంకేతిక పరిజ్ఞానంతో కూడిన ఈఎంఆర్‌ వినియోగించడంలేదు. పేపర్లపై రాసిన డేటా ఆధారంగానే నిర్ణయాలు తీసుకుంటారు. ఫలితంగా చికిత్సపై సరైన సమయంలో నిర్ణయం తీసుకోవాల్సిన వైద్యుల వద్దకు ఈ సమాచారం సకాలంలో చేరదు. ఫలితంగానే మెడికల్‌ ఎర్రర్స్‌ చోటుచేసుకుంటాయి. ఉదాహరణకు ఒక నర్సింగ్‌ టీం డ్యూటీ ముగిసే సమయంలో ఒక పేషెంట్‌కు సంబంధించిన రక్త పరీక్షలో ‘డెడ్లీ బ్యాక్టీరియా’ ఉందని రిపోర్టు వస్తే.. వచ్చే టీము చూసి వైద్యులకు అందజేస్తారని వెళ్లిపోతారు. తర్వాత డ్యూటీకి వచ్చిన వారికి ఆ రిపోర్టు చూడడంలో.. ఆ సమాచారాన్ని వైద్యులకు చేర్చడంలో ఆలస్యం కావొచ్చు. అప్పుడు ‘ఈ-కొలి’ బ్యాక్టీరియా ఉందని ట్రీట్‌మెంట్‌ ప్రారంభించినా పేషెంట్‌ను సాధారణ స్థితికి తీసుకురావడం కష్టమవుతుంది. ఎందుకంటే ‘ఈ-కొలి’ బ్యాక్టీరియాకు నాలుగైదు గంటల సమయం దొరికితే శరీరమంతా వ్యాపిస్తుంది. అదే డిజిటల్‌ ప్లాట్‌ఫాం ఉంటే.. డాక్టర్లందరి మొబైల్‌ ఫోన్‌కు ఈ సమాచారం వస్తుంది. వెంటనే సంబంధిత డాక్టర్‌ లేదా అందరు డాక్టర్లు చూస్తారు.

Narayana Hrudayalaya has been using AI for years, right? What are your experiences
AI needs digital information. Structured data is also needed. To know what is going on with the patient.. Diagnosis is the most important thing in healthcare. Human intelligence can examine ten to 15 data points at a time. But AI can analyze even 60 to 100 data points. We started a digital platform eight years ago in our hospital. Narayana Hrudayalaya's team of 50 software engineers has developed AI tools. 20 doctors in our team have this app. For example, when 'deadly bacteria' is detected, the information goes to the doctors whether they are on duty or not. The doctor at the patient's bed will definitely see. If necessary, other doctors will also respond. The activity will start immediately. Also blood gas tests are done every few hours in the ICU. The results of this test will come to the app on the phones of 20 doctors and nurses in our team. Usually only few nurses can interpret this report. A little complicated. In our hospital, along with the blood gas report, interpretation is also provided by AI. Now everything has become an expert. If there is an argument that AI will make doctors and nurses useless... on the contrary, AI has made them smarter. A smart EMR leads to better, smarter diagnosis. In the near future, within five to ten years, taking a second opinion from an AI will definitely become a legal requirement.
Heart rate increases. BP decreases. Urine output gradually decreases. Data points were analyzed for 67 different factors prior to cardiac arrest. This is where AI plays a key role. AI is just a tool. But the doctor does not always have enough time. For example, when I see an angiogram, my perspective as a heart surgeon is towards bypass surgery. When a cardiologist sees the same angiogram, they think about angioplasty. If it is not possible, it will be sent to bypass. Ultimately the agent is not the AI that decides what to do. That won't happen though. At all times only the doctor decides.
How should AI be used in the medical field?
Young children should do homework. If you ask for chat GPT.. it is misuse. Also, some doctors do what the AI tells them to do without using their brain. Apart from that, doctors should understand the analysis of AI and apply it to real situations and take decisions.
What do you say about the opinions that AI is not very useful for patients in rural areas except for those in major cities?
Currently AI is limited only to big hospitals that can spend heavily on digital healthcare and digital platforms. Hospitals without a robust digital platform cannot leverage AI. It takes time.
Who is responsible if AI prescribes wrong medicine?
The doctor is responsible. One thing to remember is that AI is only a tool to help doctors. Prevents doctors from making mistakes. Refers to an alternative treatment that does not come to our mind. What people need to understand is that AI can provide safer healthcare to patients. What is the importance of electronic medical records (EMR) in medical treatment? Even in such a place there is scope for medical error. For example, if there are 200 patients in one night in a hospital, one of them dies due to a medical error. Even in America, many hospitals do not use EMR with advanced technology. Decisions are made based on data written on papers. As a result, this information does not reach the doctors in time to make timely decisions on treatment. As a result, medical errors occur. For example, at the end of the duty of a nursing team, if a patient's blood test shows a report of 'deadly bacteria', the team will see and hand it over to the doctors. Those who come on duty later may be delayed in seeing the report and adding that information to the doctors. Then it is difficult to bring the patient back to normal even if treatment is started for the presence of 'E-coli' bacteria.
